Does anyone know if there is an actual shirt that this pattern was based off of?

Period impressions patterns are not entirely reliable as far as patterns go.  You will find that many of them don't really work out.  Pattern pieces may be missing, instructions scanty at best and pattern pieces will not always fit together.  On top of that, pattern shapes may have been changed from the original garment.  While I don't doubt that the pattern is based off a shirt period impressions saw or handled at some point I doubt the pattern they produced will give you anything that looks like the original shirt.  I would stay away.
